How it works

  1. The form is open above. Click a field outlined in blue and type; Tab goes to the next one.
  2. Click a box to tick it.
  3. Sign adds your signature, and Date adds today's date, wherever you click.
  4. Download the filled copy. Untick Flatten, under Values, to keep the fields editable.

It runs in this tab on pdfedge's own code. Nothing you type is uploaded, and closing the tab leaves nothing behind.

About this form

The form a US business asks for before it pays you as a contractor or vendor: your name, tax classification, address and taxpayer identification number.

This is the IRS's own PDF, Rev. March 2024, as published at irs.gov. The IRS explains who needs it and how to fill it in on its Form W-9 page, which also lists any newer revision: check before you send it.

pdfedge isn't the IRS and doesn't give tax advice.
